Case Management Coordinator – Telephonic, Community

This is a fully remote position, open to applicants in Illinois.

📋 Description

• The Case Management Coordinator employs critical thinking and judgment to collaborate and contribute to the case management process.

• This role facilitates optimal healthcare outcomes for members by assisting with appointment scheduling and identifying and helping members access benefits and education through care management tools and resources.

• Evaluation of Members: Conducts thorough assessments of referred members’ needs and eligibility, recommending strategies for case resolution.

• Identifies high-risk factors and service requirements that may affect member outcomes and components of care planning.

• Coordinates and executes assigned care plan activities while monitoring the progress of the care plan.

• Enhancement of Medical Appropriateness and Quality of Care: Consults with case managers, supervisors, Medical Directors, and/or other health programs to address barriers to achieving goals and objectives.

• Provides coaching, information, and support to empower members in making ongoing independent medical and/or healthy lifestyle decisions.

⛳️ Requirements

• A Bachelor’s degree or a non-licensed master’s level clinician is required, with either degree being in behavioral health or human services.

• Proficiency with computers is necessary to navigate internal and external computer systems, including Excel and Microsoft Word.

• Experience in case management and discharge planning is preferred.

• A minimum of 2 years’ experience in behavioral health, social services, or a related field relevant to the program focus is required.

• Managed Care experience is preferred.

• Strong communication, telephonic, and organizational skills are essential.

• Excellent analytical and problem-solving abilities are required.

• Capability to work independently is necessary.

• Effectively participating in a multi-disciplinary team involving both internal and external participants is essential.
