• Organize and maintain case files, records, service plans, and related documentation while ensuring adherence to compliance, confidentiality, and secure handling of sensitive information.
• Schedule and coordinate appointments, referrals, service authorizations, annual reviews, and follow-ups to guarantee timely access to services and support.
• Serve as a liaison among clients, families, service providers, and community organizations, facilitating communication in both English and Spanish as required.
• Assist in the development, monitoring, and updating of individualized service plans, tracking deadlines, renewals, and progress to keep the case management team informed.
• Oversee compliance with state and agency-specific regulations concerning service delivery and documentation standards.
• Collaborate with healthcare providers, therapists, and service coordinators to ensure clients receive comprehensive, person-centered care.
• Prepare and submit reports on client progress, service utilization, and outcomes to uphold compliance and transparency.
• Support case managers in resolving care-related issues and addressing obstacles to accessing necessary services.
• Communicate effectively with Spanish-speaking clients and families, offering interpretation and translation support to ensure equitable access to services.
• Provide insights to enhance workflows, service delivery processes, and overall client satisfaction.
• 2 years of demonstrated experience as a Case Management Assistant or in a comparable administrative role.
• Experience with scheduling, documentation, reporting, and coordination tasks in a healthcare or social services environment.
• Bilingual – Spanish and English proficiency required (both written and verbal).
• Experience working with US-based clients is a necessity.
• Strong organizational abilities with a keen attention to detail.
• Proficiency in case management software, Microsoft Office, or similar applications.
• Excellent interpersonal and communication skills with a client-centered approach.
• Ability to maintain strict confidentiality and manage sensitive information with professionalism.
• Capacity to work both independently and collaboratively in a dynamic environment.
• Starting rate: $7/hour (commensurate with experience and qualifications)
