This is a fully remote position, open to applicants in New York, +2 more states.
• Administer the annual Health Risk Survey to aid in identifying the needs for the member’s Individual Plan of Care.
• Notify the assigned care manager of any newly discovered health or safety risks, as well as service requirements.
• Execute care coordination tasks assigned by the care manager within the specified timeframe.
• Alert the assigned care manager and/or associate manager to any detected quality of care concerns.
• Diligently assist with the member’s care coordination requirements and seek solutions to meet those needs.
• Employ problem-solving abilities to locate alternative contact details for members who are not reachable by care management.
• Follow case management and quality management protocols in accordance with regulatory and accreditation standards, as well as company policies.
• A minimum of 2 years of experience in behavioral health, social services, or a related field pertinent to the program's focus.
• Competency in Microsoft Office Suite (Word, Excel, Outlook, OneNote, Teams).
