Caregiver Coach

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide high-quality, individualized coaching to family caregivers of individuals affected by dementia, customized to meet their specific challenges.

• Utilize Ceresti’s proprietary technology platform to oversee daily operations, monitor caregiver progress, and record caregiver interactions.

• Initiate and sustain regular outbound communication to build trust and foster strong, long-lasting relationships with assigned caregivers.

• Educate caregivers about the dementia disease process, common behavioral symptoms, safety strategies, and methods for managing stress and preventing burnout.

• Assist caregivers in navigating emotionally challenging situations, such as end-of-life discussions, transitions to facility care, or family disputes regarding care decisions.

• Support caregivers in care planning, preparing for appointments, accessing community resources, and coordinating care, ensuring effective communication with healthcare providers and navigating the healthcare system.

• Achieve or surpass performance metrics related to caregiver engagement, satisfaction, and program outcomes.

• Participate in regular team meetings, training sessions, and case reviews, sharing insights and best practices with colleagues and leadership.

• Engage in quality improvement initiatives, including pilot studies, internal feedback loops, and evaluations of caregiver experiences.

• Additional responsibilities as assigned.

⛳️ Requirements

• Minimum of 2 years of professional experience working with older adults or individuals living with dementia.

• Direct caregiving experience (either personal or professional) with individuals experiencing cognitive impairment.

• Proficiency in English.

• Eligibility to work in the U.S. is required.

• Capability to work independently in a fully remote environment.
