Care Navigator, Bilingual Spanish

This is a fully remote position, open to applicants in United States.

📋 Description

• Respond to incoming calls, emails, and text messages from patients, caregivers, families, external providers, and other facilities.

• Organize and maintain communication with patients through phone or email.

• Clarify health insurance benefits and eligibility to patients, and process copays and coinsurance as required.

• Conduct follow-ups after appointments to provide further instructions and coordinate care.

• Under clinical supervision, inquire about screening questions and document patient safety, behavior, functionality, medications, care requirements, and advanced care plans.

• Under clinical supervision, inquire about screening questions and document issues faced by caregivers, such as burden, depression, and poor coping mechanisms.

• Report screening outcomes and escalate any concerns to the clinical team (nurse, social worker, doctor).

• Distribute educational materials and information regarding care with Isaac and other local community-based service options as specified in the patient’s care plan.

• Foster a compassionate environment by offering emotional and psychological support to patients, caregivers, and families.

• Provide comfort by anticipating anxieties, offering supportive resources, and addressing questions within the scope of practice.

• Ensure participant privacy in compliance with HIPAA regulations.

• Collaborate within and contribute to a supportive team dedicated to our mission.


⛳️ Requirements

• Previous experience in coordinating or navigating care for patients, caregivers, and their loved ones (bonus points for experience with cognitive challenges).

• Authentic empathy, patience, outstanding time management, and communication skills.

• Personal computer or laptop that meets security and telehealth standards (SOC2).
