Care Navigator

This is a fully remote position, open to applicants in Arizona, +4 more states.

📋 Description

• Conduct Health Screenings & Intake Calls: Engage with patients via phone to perform health screenings and intake sessions using a standardized set of questions. Establish rapport quickly to ensure patients feel at ease sharing accurate details during each 15–20 minute conversation.

• Schedule & Follow Up: Arrange intake appointments directly in Healthie (no separate booking link required). If a patient is unable to complete the intake immediately, take responsibility for scheduling it yourself to maintain a consistent relationship. Follow up to confirm appointments and enhance attendance rates.

• Connect Patients to Resources: Utilize open-ended questions to ascertain each patient's needs. After completing the intake, schedule the appropriate next steps, such as a consultation with their doctor or a Community Health Worker.

• Maintain Accurate Records: Input patient information into our records system in real time during each call. Review notes for accuracy and ensure all documentation is completed by the end of your shift. Adhere to all privacy regulations (HIPAA) and obtain formal patient consent for every call.

• Achieve Your Goals: Organize your daily schedule effectively and consistently meet your targets, including maintaining a show rate of 60% or more for scheduled intakes. Space out appointments to allow ample time for reviewing notes thoroughly.

⛳️ Requirements

• Education: A high school diploma or equivalent is required. A degree in Human Services, Public Health, or Behavioral Health is advantageous.

• Bilingual: Proficiency in both English and Spanish (or another language) is highly preferred.

• Experience: Background in healthcare, social services, care platforms like Healthie, or community outreach is beneficial but not mandatory. You should be comfortable communicating with a diverse range of patients over the phone. No prior intake experience is necessary — full training is provided.

• Comfortable with Technology: You should be adept at using web and app-based systems and able to adapt quickly in a rapidly changing environment.

• Works Well Independently: You can effectively manage your own tasks, keep documentation organized, and achieve your goals while working remotely with minimal supervision.

• Willing to Train: You will participate in a comprehensive training program, which includes guided call practice, documentation training, and role-playing exercises. A score of 80% or higher on the final training assessment is required.

• Background Check: A drug test and background check must be successfully passed prior to starting.

• Physical Demands: To excel in this role, Care Navigators must be able to sit, stand, and walk regularly to communicate with members and handle daily tasks. You should be capable of seeing and hearing adequately to converse with members both by phone and in person, as well as answer detailed inquiries. Proficiency in using a computer to enter and update health information across various systems while remaining focused on the conversation is essential. A reliable internet connection and a quiet, private space for remote calls are required.
