This is a fully remote position, open to applicants in United States.
• Conduct a high volume of outgoing calls to engage patients and schedule healthcare services.
• Oversee care coordination across various services, including mammograms, bone density screenings, and primary care connections, ensuring successful scheduling, follow-ups, and completion of care.
• Identify and address barriers related to social determinants of health (SDoH) by linking patients to suitable community resources (e.g., transportation, housing, financial assistance, food access).
• Perform patient assessments to comprehend clinical, social, and logistical barriers to care.
• Inform patients about available healthcare services and community resources, ensuring they know how to access and benefit from them.
• Establish trust with patients and caregivers through clear, empathetic, and effective communication.
• Proactively follow up with patients to ensure care completion and address any gaps.
• Manage multiple workflows simultaneously while ensuring accuracy and attention to detail.
• Keep accurate and detailed records of all patient interactions and resource coordination in compliance with HIPAA and healthcare privacy regulations.
• Collaborate across functions with clinical, operations, and support teams to guarantee seamless patient experiences.
• Appropriately escalate issues in a timely manner when patterns, risks, or blockers are identified.
• Assist with additional projects and initiatives as required.
• A minimum of 4 years of experience in care navigation, care coordination, social work, call center operations, or patient outreach within a healthcare or service environment.
• Demonstrated ability to work independently in a high-volume, fast-paced setting.
• Ability to adapt quickly to changing workflows, tools, and priorities.
• Strong critical thinking and problem-solving capabilities.
• High emotional intelligence and empathy in patient interactions.
• Preferred experience in working with social determinants of health (SDoH) or community resource navigation.
• Excellent written and verbal communication abilities.
• Experience in managing multiple communication channels (phone, email, chat, etc.).
• Familiarity with Zendesk is a plus.
• Solid organizational skills and meticulous attention to detail.
• Capability to manage competing priorities while maintaining high performance and quality.
• Experience in a startup or high-growth environment is advantageous.
• Fluency in Spanish is preferred but not mandatory.
