Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Handle inquiries from patients and durable power of attorney (DPOA) via phone.

• Address messages from patients or caregivers received through the Curana Patient Portal.

• Aid patients in scheduling follow-up appointments with Curana Providers or specialists.

• Oversee electronic health records (EHR) to ensure patient records are precise and current.

• Organize documents needed for review or signature by providers.

• Facilitate provider orders and escalate findings as necessary.

• Maintain patient rosters for those enrolled in Advanced Primary Care Management (APCM) and Guiding an Improved Dementia Experience (GUIDE).

• Assist with Provider scheduling and prior authorizations.

• Obtain patient records and diagnostic test results.

• Professionally and courteously manage incoming calls.

• Collaborate with the Curana Interdisciplinary Care Team to ensure efficient communication within our health network.

• Perform other duties as required.

⛳️ Requirements

• High school diploma or its equivalent.

• A minimum of 1 year of experience in a medical office, Senior Living Community engagement, or similar fields.

• At least 1 year of experience with Electronic Health Record (EHR) documentation or other practice management tools.

• Comprehensive knowledge of medical terminology.

• Capability to interpret medical records, lab results, and appointment notes.

• Basic understanding of evaluating patient screening tools and the ability to identify changes over time.

• Ability to function effectively in a distraction-free environment.

• Exceptional organizational and time management skills with the capability to prioritize tasks.

• Proficient in managing multiple tasks concurrently.

• Strong computer skills and adaptability to various technology platforms.

• Previous experience with virtual triage.

• Bilingual or multilingual communication abilities.
