This is a fully remote position, open to applicants in California, +3 more states.

📋 Description

• Provide a supportive and positive experience for clients, referral sources, and external providers.

• Engage directly with clients, families, and referral sources to comprehend their needs and preferences.

• Conduct accurate and timely outbound referrals for individuals not admitted to Charlie Health.

• Collaborate closely with internal stakeholders at Charlie Health (e.g., clinical team, admissions team) as necessary to fulfill job responsibilities.

• Record all interactions with clients and referral sources in the electronic record system.

• Work in tandem with the Clinical Outreach and Partnerships teams to gain a thorough understanding of referral sources and the services they offer.

• Serve as a liaison between Charlie Health and its partners to ensure all ongoing needs are met, placing the client experience at the forefront.

• Follow established policies and procedures while achieving performance metric goals.


⛳️ Requirements

• Bachelor’s degree in health sciences, communications, psychology, social work, or a related field.

• 1-2 years of relevant work experience, particularly in healthcare, preferably in customer/patient-facing roles such as case management, discharge planning, referral relations, admissions, or outreach.

• Strong interpersonal skills, along with the ability to build relationships and listen effectively.

• Metrics- and results-driven mindset, with a history of working towards concrete targets.

• Proven track record of meeting or exceeding KPIs in previous roles.

• Excellent written and verbal communication abilities.

• Exceptional organizational skills and attention to detail.

• Authorized to work in the United States with native or bilingual proficiency in English.

• Ability to excel in a fast-paced environment and adapt quickly.

• Proficient in Salesforce and Google Suite/MS Office.
