This is a fully remote position, open to applicants in Mississippi.
• Develop, evaluate, and coordinate care management activities tailored to member needs.
• Create or contribute to individualized care plans/service plans.
• Educate members and their families/caregivers about available services, benefit options, access to care, and high-quality care standards.
• Assess member needs, identify barriers to care, evaluate available resources, and facilitate suitable care plans.
• Identify necessary providers, specialists, and community resources for effective care.
• Provide psychosocial and resource support related to employment, education, housing, food, participant direction, independent living, justice, and foster care services.
• Facilitate communication between members, families, caregivers, and care provider teams to ensure timely access to care and services.
• Monitor progress towards care plan objectives and member status; collaborate with healthcare providers on necessary revisions and identified needs.
• Collect, document, and maintain member information and care management activities to ensure compliance with regulations.
• Conduct on-site visits when necessary to assess needs and work with providers or resources.
• Educate care managers, members, and families/caregivers on procedures, provider instructions, care options, referrals, and healthcare benefits.
• Perform additional duties as assigned and adhere to policies and standards.
• Bachelor’s degree with 2–4 years of relevant experience.
• Graduate from an Accredited School of Nursing if holding a clinical license.
• Equivalent experience gained through applicable knowledge, duties, scope, and skills that reflect the level of this position may also be considered.
• For those applying for the Mississippi Magnolia Health plan: a Bachelor’s or Master’s degree in a healthcare-related field (social work, sociology, psychology, public health) along with 2–4 years of relevant experience.
• LSW or LMSW licensure is preferred.
• Experience in obstetrics is preferred.
