Care Navigator

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Engages in outreach activities to members and providers to verify service details, coordinate scheduling, respond to general inquiries, and ensure precise documentation of care coordination efforts.

• Facilitates service coordination by collaborating with providers, community partners, and members/caregivers to manage scheduling, service updates, and necessary administrative adjustments.

• Works in partnership with the care team, providers, and other stakeholders as necessary to support member service requests and maintain seamless communication and continuity in care activities.

• Assists members in arranging transportation for medical appointments and helps resolve any booking issues.

• Oversees interpreting and translation services for members, ensuring ADA compliance.

• Acts as a mentor for new Care Navigation Specialists, supporting them during and after their orientation period as required.

• Ensures accurate documentation and maintenance of member records in alignment with regulatory standards, facilitating timely distribution to relevant internal teams or provider partners as needed.

⛳️ Requirements

• High School diploma or GED is mandatory.

• At least one (1) year of relevant experience in a health-related service field is required.

• Strong critical thinking and troubleshooting skills.

• Excellent customer service and problem-solving abilities.

• Capability to operate within multiple systems, often simultaneously.

• Clear, concise, and effective verbal and written communication skills.

• Strong time management skills with a proven ability to work independently.

• Flexibility to adapt to changing business needs.

• A motivated team player.

• Outstanding organizational skills.

• High attention to detail and accuracy is essential.

• Basic arithmetic skills are required.

• Proficiency in Microsoft Office Suite.
