This is a fully remote position, open to applicants in Florida.
• Act as a primary contact for patients and their family caregivers.
• Comprehend the needs of patients and caregivers, facilitating connections with Nolia services and community resources.
• Deliver caregiver and disease education utilizing approved materials.
• Perform family check-ins and offer Care Navigation as necessary without managing a permanent caseload.
• Identify issues that necessitate clinical assessment and escalate them to a licensed clinician.
• Respond to and follow up on inquiries and calls from patients and caregivers.
• Assist with appointment scheduling, new patient intake, onboarding, eligibility verification, and enrollment processes.
• Ensure accurate patient information is maintained and that clinicians have the essential details prior to appointments.
• Provide administrative support for no-shows, patient technology concerns, scheduling requirements, and routine obstacles to care.
• Facilitate patient outreach, referrals, and follow-up activities.
• Offer daily administrative and coordination assistance to the interdisciplinary clinical team.
• Accurately document interactions with patients and caregivers.
• Adhere to documentation and time-tracking standards.
• Follow up on missing documentation, incomplete encounters, and pending tasks.
• Aid in the preparation of monthly billing and other clinical program processes.
• Uphold patient confidentiality and comply with HIPAA and relevant privacy regulations.
• A Bachelor’s degree in Social Work (BSW) or a related field within human services.
• Experience through employment, internships, or practicums in supporting patients, caregivers, or individuals dealing with health and social needs.
• Basic understanding of community resources, healthcare navigation, social services, and/or caregiver support.
• Excellent communication, interpersonal, organizational, and problem-solving abilities.
• Capability to identify when the needs of patients or caregivers need to be escalated to a clinician or supervisor.
• Comfortable operating in a dynamic, fast-paced healthcare environment and adapting to new technologies and workflows.
• MSW or prior experience in care navigation, case management, patient navigation, or care coordination is preferred.
• Experience in supporting older adults, individuals living with dementia or chronic illnesses, or family caregivers is preferred.
• Knowledge of Medicare, healthcare documentation, or care management programs is preferred.
• Maintain patient confidentiality and comply with HIPAA and applicable privacy regulations.
• Training in Nolia's Care Navigation model.
