• Conduct in-home and telehealth care management visits to evaluate and determine social and behavioral conditions.
• Collaborate closely with the Care Team to ensure effective teamwork and optimal patient outcomes.
• Evaluate social determinants of health needs and formulate a strategy to address them.
• Identify, assess, and establish relationships with local Community-Based Organizations.
• Inform patients about relevant resources, assist with referral processes, and follow up to ensure closure of outcomes.
• Act as a subject matter expert on social determinants for other members of the Care Team.
• Conduct assessments of behavioral, environmental, and social support needs.
• Provide education to individuals, families, and groups on managing chronic illnesses.
• Engage family members and social support groups in the education and care process of patients.
• Evaluate patients and refer them to behavioral health specialists when diagnosis and treatment are necessary.
• Assist patients in understanding, accepting, and adhering to medical and lifestyle recommendations.
• Review and document patient updates and progress in the care management platform.
• Currently licensed as a LCSW or LMSW in the relevant state.
• Master's degree in social work and successful completion of the ASWB masters or clinical examination.
• Basic Life Support (BLS) certification is mandatory for this position.
• Self-motivated individual with the ability to work independently with minimal oversight.
• Demonstrated empathy and capability to quickly establish relationships with patients and local CBOs.
• Preferred minimum of 2 years of experience in care management and/or working with chronic illnesses.
• Exceptional verbal communication skills, both in-person and via phone.
• Proficient in Microsoft Office, along with mobile and web-based applications.
