Care Manager – SNP

This is a fully remote position, open to applicants in United States.

📋 Description

• Facilitating the coordination of care for SNP members by developing and upholding a care plan.

• Collaborating with ICT members and managing care during transitions of care.

• Serving as a liaison for the Interdisciplinary Care Team (ICT) alongside the PCP and beneficiary/caregiver.

• Working together with other CM team members to assist the beneficiary during TOC events.

• Overseeing and prioritizing member self-referrals to care management programs.

• Aiding in the adherence to CMS SNP Model of Care (MOC) standards, NCQA criteria, and Medicare Advantage regulatory obligations.

⛳️ Requirements

• Valid Registered Nurse license issued by the state where services will be rendered or a current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).

• A minimum of three (3) years in healthcare clinical experience.

• At least one (1) year of experience in Care Management, Case Management, or Population Health.

• Bachelor's Degree in Nursing OR an Associate of Science in Nursing Degree (ASN) or Diploma.

• Experience managing Medicare and/or Medicaid and/or SNP populations.

• One (1) year of experience in SNP Care Management.
