Care Manager, RN

This is a fully remote position, open to applicants in Mississippi.

📋 Description

• Design, evaluate, and implement intricate care management activities for members primarily with physical health needs.

• Offer tailored care plans and educational resources for both members and their families.

• Assess member requirements, identify access barriers, and consider social determinants affecting health.

• Create continuous care/service plans while collaborating with healthcare providers, specialists, and community resources.

• Recognize challenges and obstacles to care and deliver necessary care management interventions.

• Serve as a liaison among members, caregivers, and care provider teams.

• Conduct follow-ups and monitor changes in member status, conditions, and progress towards care plan objectives.

• Provide resource support for various services including employment, housing, participant direction, independent living, justice, and foster care.

• Facilitate effective care management and ensure timely access to required care or services.

• Perform outreach via phone, digital platforms, home visits, and/or other locations.

• Collect, document, and preserve member information and care management activities to ensure regulatory compliance.

• Educate members and their families/caregivers regarding disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.

• Offer feedback on leadership opportunities to enhance care and quality delivery in a cost-effective manner.

• Execute additional responsibilities as assigned while adhering to policies and standards.

⛳️ Requirements

• Degree from an accredited nursing program or a Bachelor's degree in Nursing.

• 2–4 years of relevant experience.

• RN (Registered Nurse) state licensure and/or compact state licensure is required.

• Must be licensed in the state of Mississippi.

• Candidates must reside in Mississippi.

• Adherence to all policies and standards is mandatory.

• Compliance with applicable regulatory requirements is required.
