This is a fully remote position, open to applicants in Michigan.
• Develop, evaluate, and implement intricate care management strategies for members with primarily physical health needs.
• Assess member requirements, obstacles to accessing care, and social determinants affecting health.
• Create individualized care/service plans and collaborate with healthcare providers, specialists, and community resources.
• Identify challenges and barriers to care, providing suitable care management interventions.
• Coordinate efforts among members, families/caregivers, and care provider teams to ensure a person-centered approach to care.
• Monitor and follow up on member status, changes in condition, and progress towards care plan objectives.
• Revise and update care plans in collaboration with members, caregivers, and providers as necessary.
• Offer resource support for employment, housing, participant direction, independent living, justice, and foster care services.
• Facilitate care management and ensure timely access to necessary care and services.
• Conduct outreach through telephonic, digital, home visits, and/or other sites.
• Collect, document, and maintain member information and care management activities to ensure regulatory compliance.
• Educate members and their families/caregivers about disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.
• Provide leadership insights on opportunities for improving care delivery and quality in a cost-effective manner.
• Perform additional assigned duties and adhere to policies and standards.
• A degree from an accredited nursing program or a Bachelor's degree in Nursing.
• 2–4 years of relevant experience.
• An unrestricted Michigan RN license is mandatory.
• RN state licensure and/or compact state licensure is required.
• Availability to work Monday–Friday, 8 AM–5 PM Eastern Time.
