Care Manager, RN

This is a fully remote position, open to applicants in Kansas.

📋 Description

• Responsible for developing, assessing, and facilitating intricate care management activities.

• Evaluates member needs and suggests suitable care options.

• Creates ongoing care plans and collaborates with healthcare providers.

• Identifies issues and implements care management interventions.

• Coordinates care among members, caregivers, and healthcare provider teams.

• Conducts continuous follow-up and monitoring of member status.

⛳️ Requirements

• Must possess a degree from an accredited nursing school or a Bachelor’s degree in Nursing.

• Requires 2 to 4 years of relevant experience.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.
