This is a fully remote position, open to applicants in Florida.
• Design, evaluate, and implement intricate care management strategies for members primarily with physical health needs.
• Create tailored care plans and provide educational resources for members and their families.
• Assess member requirements, obstacles to accessing care, and social determinants influencing health.
• Formulate continuous care plans and collaborate with healthcare providers, specialists, and community organizations.
• Recognize issues and barriers to care while delivering care management solutions.
• Facilitate coordination among members, their families/caregivers, and healthcare provider teams.
• Monitor and follow up on member status, changes in condition, and progress towards care plan objectives.
• Update or adjust care plans in collaboration with members, caregivers, and providers as necessary.
• Offer support for resources relating to employment, housing, participant direction, independent living, justice, and foster care.
• Promote effective care management and ensure timely access to necessary services or care.
• Conduct outreach through telephonic, digital, home, and/or other site visits.
• Gather, record, and maintain member data and care management activities to ensure regulatory compliance.
• Provide or facilitate educational sessions on disease processes, care gaps, provider instructions, care alternatives, referrals, and healthcare benefits.
• Offer leadership insights on potential improvements in care and quality service delivery.
• Execute additional responsibilities as assigned while adhering to policies and standards.
• A degree from an accredited School of Nursing or a Bachelor's degree in Nursing.
• 2–4 years of relevant experience.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure is required.
• Must be a licensed RN in Florida.
• Candidates must reside in the state of Florida.
• Availability to work Monday–Friday, from 8am to 5pm Eastern Time.
• Clinical experience with chronic conditions or diseases is preferred.
• Experience in case management is advantageous.
• Background in managed care is beneficial.
• Strong time management and communication abilities.
