Care Manager, Registered Nurse, RN

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Evaluates and assesses the needs and requirements of members.

• Assists members and their families in navigating and connecting with resources that support their care and well-being.

• Utilizes various strategies, approaches, and techniques to address members' physical, environmental, and psycho-social health challenges.

• Identifies and addresses obstacles that impede care.

• Monitors patient care through assessments and evaluations to ensure progress towards desired outcomes.

• May develop care plans for members.

⛳️ Requirements

• Minimum of 3 years of clinical acute care experience.

• Must be a Licensed Registered Nurse (RN) in the state of South Carolina with no disciplinary actions.

• Extensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook.
