This is a fully remote position, open to applicants in Kansas.
β’ Responsible for developing, assessing, and implementing intricate care management activities primarily focused on members with mental and behavioral health needs.
β’ Assesses member requirements through phone consultations or in-home visits concerning available resources, recommending and/or facilitating the care/service plan for optimal outcomes.
β’ May conduct telephonic, digital, home, or other site visits to evaluate member needs and collaborate with various resources.
β’ Creates ongoing care plans for members with a high level of acuity while identifying necessary providers, specialists, and community resources for comprehensive care.
β’ Oversees care plans/service plans and/or monitors member status and outcomes for any changes in treatment side effects, complications, and clinical symptoms.
β’ Educates members and their families regarding procedures, healthcare provider instructions, treatment alternatives, referrals, and healthcare benefits.
β’ A Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing is required.
β’ 2 to 4 years of relevant experience is necessary.
β’ A Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN, based on state contract requirements with behavioral health experience is mandatory.
