This is a fully remote position, open to applicants in Florida, +3 more states.
• Execute administrative care management tasks such as member outreach, handling inbound calls, and scheduling services.
• Act as a primary contact for members, providers, and staff to address and resolve issues.
• Maintain member documentation in alignment with state and regulatory requirements.
• Reach out to members via phone to discuss care-plan next steps, community or health-plan resources, scheduling inquiries, concerns, and ongoing educational needs.
• Link members with health-plan and community resources to ensure high-quality customer service.
• Offer first-line support for inquiries, requests, and concerns from both members and providers.
• Clarify care-plan procedures and protocols to members.
• Assist with member onboarding and administrative tasks, which include sending welcome letters, correspondence, and educational materials.
• Keep and maintain non-clinical member records, providing them to providers as necessary.
• Make referrals to meet Social Determinants of Health needs.
• Carry out other assigned responsibilities while adhering to company policies and standards.
• High School diploma or GED is required.
• 1–2 years of relevant experience is necessary.
• Preferred: At least 2 years of experience supporting adults aged 21 and older receiving Long-Term Services and Supports (LTSS).
• Preferred: A minimum of 2 years in member or provider outreach within a fully outbound call environment.
• Preferred: Over 2 years of experience processing healthcare/insurance authorizations and coordinating member or provider services.
• Must be detail-oriented, adaptable, and a hands-on learner.
• Proficiency in web-based and CMS systems is required.
• Must be available to work Monday–Friday, from 8:00 am to 5:00 pm or 8:30 am to 5:00 pm EST.
