Care Management Scheduler

This is a fully remote position, open to applicants in United States.

📋 Description

• Patient Outreach & Education: Make calls to eligible patients, effectively communicating the advantages of our care programs aimed at managing chronic illnesses and enhancing overall health. Support the care team with patient communication.

• Eligibility Verification: Validate patient eligibility and confirm insurance coverage.

• Data Entry & Management: Ensure accurate and current records of patient interactions, maintaining documentation to uphold the integrity of the patient’s chart.

• Patient-Centered Communication: Engage in a professional, empathetic, and compelling manner to foster trust and promote participation.

• Team Collaboration: Collaborate closely with internal teams and healthcare providers to guarantee seamless coordination and support.

• Performance Goals: Regularly achieve or surpass set enrollment objectives.

• Previous experience in a healthcare environment is preferred.

• Proficient in basic computer applications and data entry.

• Strong problem-solving abilities and effective multitasking skills.

• Exceptional verbal and written communication skills.

• A collaborative team player with a positive mindset.

• Capability to work independently in a remote setting.

• Successful completion of a background check and drug screening is required.

• Experience using various EMR systems and payer portals.
