This is a fully remote position, open to applicants in New York.
• Collaboratively develop and implement patient-centered care plans and objectives alongside the client/family and other healthcare providers.
• Work in tandem with team members to facilitate outreach and engage with patients.
• Collaborate closely with in-office providers and staff to deliver patient education that aids in self-management.
• Identify gaps or obstacles within treatment plans.
• Manage care for enrolled patients, which includes scheduling appointments.
• Oversee referrals as necessary.
• Coordinate community resources when required (e.g., home health, durable medical equipment, etc.).
• Provide education to members regarding disease processes.
• Motivate members to adopt healthier lifestyle choices.
• Actively manage a designated caseload of patients.
• Conduct outbound calls to evaluate members' current health status.
• Receive and respond to incoming calls from patients, gathering health data and offering medical guidance under the direction of the patient's provider.
• Document calls in accordance with established protocols.
• Connect the patient's care team with updated information as it becomes available.
• Participate in educational and in-service programs.
• Perform on-call duties for the assigned region.
• Carry out additional tasks as assigned.
• Must possess an unrestricted and current license to practice as LVN/LPN/RN in New York State.
• A minimum of 3 years of experience in a Care Management setting or as a Case Coordinator within homecare, inpatient, physician practice, or in-home case management environments.
• Strong understanding of chronic disease states, including chronic kidney disease, diabetes mellitus, chronic obstructive pulmonary disease, and congestive heart failure, as well as basic medical management of these conditions.
• Highly motivated and results-oriented, with strong skills in presentation, communication, organization, and time management.
• Excellent organizational and interpersonal abilities.
• Exceptional customer service skills, evidenced by positive feedback from patients and team members.
• Capability to identify problems and propose solutions.
• Demonstrates advanced proficiency in using available computer technology, including typing skills.
• Exhibits leadership, communication, interpersonal, and problem-solving skills.
• Experience with Electronic Health Records (EHR).
• Ability to sit and work at a computer for extended periods, typing at a speed of 60 words per minute.
