This is a fully remote position, open to applicants in United States.
• Perform monthly clinical check-ins to identify and address any clinical concerns.
• Reinforce care plans and escalate issues as necessary.
• Ensure the accuracy of clinical documentation in both the EHR and partner technology platforms.
• Foster strong, ongoing relationships with patients and caregivers through consistent outreach.
• Conduct structured assessments for symptoms such as pain, fatigue, neuropathy, shortness of breath, and nausea.
• Utilize licensed clinical judgment to evaluate changes in patient conditions.
• Assess issues related to medication adherence, side effects, or confusion regarding treatment regimens.
• Identify clinical red flags that require escalation, including worsening symptoms, new shortness of breath, uncontrolled pain, or concerning vital signs.
• Collaborate with the provider team regarding changes in symptoms, new concerns, or medication-related issues.
• Clearly document all patient interactions in both the EHR and CCM patient management platform.
• Accurately track and log qualifying CCM time for compliance with billing standards.
• Active LPN or Licensed Vocational Nurse (LVN) license is required.
• A minimum of 1–2 years of clinical experience in:
• Ambulatory care.
• Population health.
• Care coordination.
• Case management.
• Strong skills in clinical assessment, communication, and documentation.
• Comfortable using two systems (EHR and CCM platform) concurrently.
• Ability to manage a structured, metric-driven workflow with consistency.
• Intermediate proficiency with MS Office Suite, including Outlook and Teams.
• Capable of working effectively in a primarily remote environment:
• Home internet connection must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps; Cable, Fiber, or DSL connections hardwired to the internet device are recommended.
