This is a fully remote position, open to applicants in New Hampshire.
• Offer administrative assistance to the Foster Care/DCYF team.
• Support Care Managers and Care Navigators in emailing and updating monitoring care plans.
• Maintain the assigned monitoring care plans.
• Conduct member outreach to facilitate comprehensive assessments.
• Assist in care management activities and support assigned member teams.
• Act as a liaison between care managers and providers to coordinate member care.
• Perform member outreach via phone or home visits.
• Discuss care plans/service plans, next steps, resources, questions, concerns, and ongoing education with members.
• Coordinate care activities based on care/service plans established.
• Address inquiries, requests, and concerns from members and providers.
• Handle some complex or escalated care management issues.
• Communicate with care managers, practitioners, and others to ensure member services and continuity of care.
• Conduct service assessments/screening and document member care needs.
• Maintain and document member records, distributing them to providers as necessary.
• Assist the care management team in triaging, adjusting, and escalating complex requests.
• Identify needs and refer to Care Managers, community-based organizations, and Disease Managers.
• Provide education regarding benefits and resources.
• Support training and development initiatives.
• Perform additional duties as assigned.
• Adhere to all policies and standards.
• High School diploma or GED is required.
• 2–4 years of relevant experience preferred.
• Capability to assist care management activities and support assigned member teams.
• Ability to perform member outreach via phone or during home visits.
• Proficiency in coordinating care with healthcare and community providers, partners, members, and caregivers.
• Ability to conduct service assessments/screening for members with complex needs.
• Skill in documenting and maintaining member records per state and regulatory requirements.
• Competence in triaging, adjusting, and escalating complex requests.
• Familiarity with contractual requirements, regulatory guidelines, standards, policies, and best practices.
• Ability to identify needs and make appropriate referrals.
