Care Coordinator II

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Assist in care management initiatives and collaborate with designated care teams to guarantee provider-delivered services, continuity of care, and member satisfaction.

• Conduct outreach to members via phone or home visits.

• Engage with members about care plans/service plans, next steps, resources, inquiries, concerns, and ongoing education.

• Coordinate care activities with healthcare professionals, community providers, partners, members, and caregivers.

• Address member and provider inquiries, requests, and concerns related to care plans/service plans.

• Communicate with care managers, practitioners, and others to facilitate member services and maintain continuity of care.

• Support service assessments/screenings and document member care needs.

• Record and manage member documentation and distribute records to providers as required.

• Identify needs and refer to Care Managers, community-based organizations, and Disease Managers.

• Educate members on available benefits and resources.

• Perform additional duties as assigned and adhere to all policies and standards.

⛳️ Requirements

• High School diploma or GED.

• 1–2 years of relevant experience.

• Previous experience in foster care is preferred.

• Capability to work remotely from home anywhere in Missouri.

• Availability from Monday to Friday, 8 am to 5 pm.

• Ability to conduct member outreach through phone calls and home visits.

• Proficiency in documenting and maintaining member records in compliance with state and regulatory requirements.

• Ability to adhere to contractual, regulatory, and organizational policies and standards.
