Care Coordinator II

This is a fully remote position, open to applicants in Kansas.

📋 Description

• Assist with care management initiatives and designated member teams to guarantee provider services, continuity of care, and member satisfaction.

• Engage in member outreach through telephone calls and home visits.

• Review care plans and service plans, discuss next steps, resources, inquiries, concerns, and ongoing education with members.

• Coordinate care activities with healthcare and community providers, partners, members, and caregivers.

• Address member and provider inquiries, requests, and concerns pertaining to care plans and service plans.

• Communicate effectively with care managers, practitioners, and others to enhance member services and ensure continuity of care.

• Assist with service assessments and screenings and record member care needs.

• Maintain member records in compliance with state and regulatory requirements and share them with providers as necessary.

• Adhere to contractual, regulatory, and organizational standards and policies.

• Recognize needs and refer to Care Managers, community-based organizations, and Disease Managers.

• Provide information on available benefits and resources.

• Execute other assigned tasks and comply with all policies and standards.

⛳️ Requirements

• A High School diploma or GED is required.

• 1–2 years of relevant experience is required.

• Must be a resident of the state of Kansas.

• Capability to conduct telephonic outreach and home visits.

• Ability to coordinate care with healthcare and community providers, partners, members, and caregivers.

• Competence in identifying needs and making referrals.

• Proficiency in documenting and maintaining member records in accordance with state and regulatory requirements.

• Ability to comply with contractual requirements, regulatory guidelines, standards of practice, and organizational policies.
