Care Coordinator II

This is a fully remote position, open to applicants in Kansas.

📋 Description

• Assists with care management activities and collaborates with teams responsible for members to guarantee the delivery of services.

• Conducts member outreach via phone or through home visits and records the care/services plan of activities.

• Orchestrates care activities according to the care plan/service plan and engages with healthcare and community providers.

• Interacts with care managers, practitioners, and other stakeholders as necessary to streamline member services.

• Adheres to all policies and standards in place.

• Must possess a High School diploma or GED.

• Requires 1 to 2 years of relevant experience.
