Care Coordinator, CCM/RPM

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee a monthly distribution of patients, ensuring that every patient assigned to you receives appropriate care.

• Enhance HealthXL's CCM/RPM platform to maintain consistent and compliant service delivery.

• Efficiently navigate various platforms (CCM/RPM system, relevant EMR, and 8x8, among others) and accurately log all information collected each month.

• Make several attempts to contact each patient monthly and create/update their personalized Care Plan.

⛳️ Requirements

• Clinical certifications, including CMA, RMA, LPN, or other approved qualifications.

• At least 1 year of experience in a clinical or patient care environment.

• Excellent verbal communication skills, exhibiting empathy.

• Strong critical thinking abilities.

• Organized with meticulous documentation skills.

• Reliable: Self-motivated, capable of prioritizing tasks, and possessing effective time management skills.

• Comfortable engaging with and evaluating the status of patients remotely, and an active listener.
