
Care Coordinator β Care Gap Closure Program

This is a fully remote position, open to applicants in United States.
β’ Perform high-volume outreach calls to healthcare providers to address care gaps and facilitate patient care coordination.
β’ Establish and maintain strong relationships with healthcare providers through consistent phone communication.
β’ Navigate intricate phone discussions with medical offices and health systems.
β’ Implement both warm and cold calling campaigns to engage providers in our programs.
β’ Arrange and conduct follow-up calls to ensure the success of care coordination.
β’ Professionally and expertly manage inbound inquiries from providers.
β’ Effectively communicate with clinical teams via phone and email to resolve care-related issues.
β’ Present information in a clear and persuasive manner to encourage provider involvement.
β’ Accurately document call outcomes and provider interactions, adhering to quality standards.
β’ Ensure the precise updating of patient and provider information in databases.
β’ Monitor outreach metrics and track the success rates of programs.
β’ Maintain follow-up schedules and calendars efficiently.
β’ Initiate requests for specialist recommendations and follow up to clarify any questions.
β’ Identify alternative contact details and escalate any unresolved outreach barriers.
β’ Utilize designated escalation dispositions to highlight issues for supervisor or clinical review.
β’ Maintain an accurate status of call queue availability.
β’ Collaborate across current and future CCGC lines (e.g., SUPD, PolyACH) and assist in scaling pilot programs.
β’ Execute additional duties as assigned.
β’ At least 2 years of experience in healthcare administration, or an equivalent combination of education and relevant work experience.
β’ High school diploma or equivalent (mandatory).
β’ Certification as a Medical Assistant or higher (preferred).
β’ Experience in the healthcare or insurance sectors (preferred).
β’ Strong understanding of medical terminology.
β’ Comprehensive knowledge of HIPAA regulations and a commitment to compliance.
β’ Outstanding organizational skills, meticulous attention to detail, and effective time management abilities.
β’ Proficient in data entry, documentation, and creating professional templates.
β’ Excellent written and verbal communication skills, along with strong interpersonal abilities.
β’ Strong problem-solving skills.
β’ Proficiency in Google Workspace (required);
β’ Experience with PowerBI (a plus).
β’ Reliable, high-speed internet access with a minimum of 100 Mbps download and 20 Mbps upload speeds.
β’ A quiet, dedicated workspace that complies with HIPAA regulations.
β’ Standard Monday to Friday work schedule; 8:00 AM - 5:00 PM local standard time.
β’ Coverage during some non-major holidays will be required.
