Care Coordinator

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Assist in scheduling for Complex Care Navigators

• Conduct patient reminder phone calls

• Promote collaboration with health plan care managers

• Ensure accurate documentation of all patient interactions

• Aid outreach and navigation teams with both operational and non-clinical responsibilities

• Handle patient calls and effectively transfer escalated issues, ensuring the involvement of appropriate resources

• Identify and request essential patient documentation from both patients and community providers

• Additional duties may be assigned

⛳️ Requirements

• High School Diploma / GED (or equivalent)

• Experience in resolving escalated customer service issues

• Proficient in Microsoft Office Suite – including Microsoft Word (for creating and editing correspondence), Microsoft Outlook (for email and calendar management), Microsoft PowerPoint (for creating and editing presentations), and strong skills in Microsoft Excel (for creating, editing, and sorting spreadsheets, along with basic analytical formulas such as VLOOKUP and IF)

• Familiarity with phone and fax systems

• Experience with Electronic Medical Records (EMR)

• Associate's Degree or higher (Preferred)

• Typing speed of 40+ WPM (Preferred)

• Background as a Medical Assistant, Certified Nursing Assistant, Pharmacy Technician, or other clinical or administrative roles within a healthcare environment (Preferred)
