Care Coordinator

This is a fully remote position, open to applicants in Philippines.

📋 Description

• Serve as the primary communication link among patients, healthcare providers, specialists, and multidisciplinary care teams.

• Assist in the creation, updating, and monitoring of personalized patient care plans.

• Monitor progress towards clinical goals and arrange necessary follow-up appointments.

• Ensure accurate maintenance of electronic health records (EHR/EMR).

• Manage referrals and conduct insurance eligibility checks.

• Address incoming patient inquiries through phone, email, or chat.

• Connect patients with community resources, home care services, medical equipment suppliers, and financial assistance programs.

• Perform regular patient check-ins to assess treatment adherence and identify care barriers.

• Provide education to patients regarding their prescribed care plans.

• Ensure that coordination activities comply with HIPAA regulations, patient privacy laws, and standard operating procedures (SOPs).

• Identify delays in treatment, gaps in care, and persistent operational issues.

• Promptly escalate issues to clinical leadership.

⛳️ Requirements

• Minimum of 3 years of experience in healthcare coordination, medical administrative support, patient navigation, or case management.

• Proficient in using Electronic Health Record (EHR) systems.

• Skilled in utilizing HIPAA-compliant communication tools.

• Competent in Microsoft Office Suite and Google Workspace.

• Exceptional verbal and written communication abilities.

• Strong empathetic listening skills.

• Excellent time-management capabilities.

• High attention to detail.

• Demonstrated ability to manage multiple care tracks concurrently.

• Preferred: previous experience in a remote healthcare or telehealth setting.

• Preferred: knowledge of medical billing, coding, or insurance verification.

• Preferred: bilingual skills as required by the patient demographic.

• Strong problem-solving and conflict-resolution skills.
