Care Coordinator

This is a fully remote position, open to applicants in Philippines.

📋 Description

• Assist older adults and their caregivers through the intake process with compassion, clarity, and professionalism.

• Connect patients with the appropriate therapist to ensure they feel secure and understood.

• Be the reliable guide supporting each new patient as they embark on their journey into care.

⛳️ Requirements

• Compassionate and articulate communicator. You engage with kindness, listen attentively, and help others feel comfortable – even during challenging or emotional discussions.

• Exceptionally organized and detail-focused. You can manage multiple patients at various stages in the process and ensure nothing falls through the cracks.

• Tech-savvy. You are comfortable learning tools such as Google Workspace, EHR platforms (e.g., Healthie), or CRMs like HubSpot, and you quickly adapt to new software.

• Aligned with our mission. You have a strong passion for enhancing care for older adults and are eager to play a significant role in someone’s initial steps toward improved mental health.

• Prior experience is welcome but not essential. You may have a background in healthcare, customer service, or an administrative support role – but most importantly, you are organized, empathetic, and motivated to grow.
