Care Coordinator

This is a fully remote position, open to applicants in Canada.

📋 Description

• Assist older adults and their caregivers throughout the intake process with compassion, clarity, and professionalism.

• Support each new patient as they embark on their care journey, ensuring they are matched with the appropriate therapist and feel understood and confident at every stage.

• Offer emotional support and reassurance throughout the intake experience.

• Effectively handle multiple patient interactions while fostering a supportive atmosphere during potentially stressful situations.

⛳️ Requirements

• An empathetic and articulate communicator. You engage with kindness, listen attentively, and help others feel comfortable, even in challenging or emotional discussions.

• Exceptionally organized and detail-oriented. You can manage multiple patients at varying stages of the process and ensure that nothing falls through the cracks.

• Comfortable with technology. You are proficient in tools like Google Workspace, EHR platforms (such as Healthie), or CRMs like HubSpot, and you quickly adapt to new software.

• Aligned with our mission. You are genuinely passionate about enhancing care for older adults and are eager to play a significant role in their journey toward improved mental health.

• Experience is a plus but not mandatory. You may have a background in healthcare, customer service, or administrative support, but what truly matters is your organization, compassion, and willingness to develop.
