This is a fully remote position, open to applicants in Maryland.
• Provide administrative assistance to the members of the Care Management (CM) team.
• Gather data for Health Risk Assessments (HRA).
• Evaluate eligibility and benefits.
• Identify members lacking a Primary Care Provider (PCP) and refer them to Member Services.
• Prioritize and screen members for CM services, referring as necessary.
• Carry out transition of care responsibilities, which include contacting the members’ PCP, Medical Power of Attorney (POA), or other medical providers for information regarding special needs.
• Document all interactions and communications made on behalf of clients, complete and submit monthly reports, and maintain detailed electronic member records.
• Encourage members to actively engage in their health and overall wellness.
• Provide educational support, member follow-ups, arrange PCP visits, and conduct care coordination under the guidance of the Care Manager.
• Assist members in evaluating health-related services, which includes obtaining a medical home, offering guidance on the appropriate use of the medical home, overcoming barriers to necessary medical care, and/or accessing social services.
• Execute tasks as instructed by the CM team to enhance member compliance, such as verifying appointments or obtaining medical records.
• Facilitate member access to community resources, including, but not limited to, housing, food, clothing, prenatal classes, parenting education, and relevant mental health services.
• Collaborate and function effectively within a team environment.
• Establish positive, supportive relationships with participants and provide constructive feedback to other team members.
• Build and sustain positive working relationships with members, providers, nurse care managers, social workers, agency representatives, and office staff from various cultural and socio-economic backgrounds.
• Excellent interpersonal, communication, and customer service abilities.
• Familiarity with community agencies and resources.
• Understanding of medical terminology.
• Working knowledge of multi-system outreach programs relating to health care delivery, clinical education, and health-related services.
• Awareness of transportation and other barriers to care that members may encounter.
• Capability to convey medical information to healthcare professionals and members of the care management team.
• Proficiency in computer applications, particularly Microsoft Office.
• High school diploma or equivalent qualification.
• Over 2 years of experience serving the Medicaid population, preferably within the MD/DC area.
• More than 2 years of experience in OB/GYN.
