This is a fully remote position, open to applicants in Texas.
• Act as a key contact and educational resource for patients, families, providers, and staff
• Conduct telephonic follow-up with members, focusing on post-hospital discharge and chronic condition management
• Complete designated Care Coordination responsibilities
• Create tasks for the assigned RN Case Manager
• Guide patients and families towards suitable resources, including community-based options
• Support, under the guidance and supervision of the RN Case Manager, with member goals, education, resource access, and follow-up care arrangements
• Ensure high-quality service in line with organizational standards
• Assist with data entry, faxes, filing, and other administrative tasks
• Manage inbound and outbound calls, gathering information as required by licensed personnel
• Over 1 year of relevant work experience in a healthcare setting
• High school diploma or GED equivalent
• Exceptional organizational and time management abilities
• Strong verbal and written communication skills
• Capable of communicating effectively over the phone in a professional manner
• Proficient in Microsoft Office Suite, including Excel, Word, Outlook, and PowerPoint
• Ability to handle multiple tasks in a dynamic environment
