This is a fully remote position, open to applicants in United States.
• Oversee a designated caseload of patients needing behavioral health assistance
• Perform regular patient outreach and follow-up to assess patient status and requirements
• Reinforce care and treatment plans, ensuring adherence and promoting patient involvement
• Recognize and report any alterations in condition, safety issues, or obstacles to care
• Communicate effectively with patients, their families, and caregivers when necessary
• Collaborate with interdisciplinary teams including healthcare providers, nursing personnel, social services, and facility management
• Aid in the coordination of behavioral health services, including scheduling and follow-up care
• Facilitate communication to guarantee continuity and coherence of care plans
• Document patient interactions and coordination efforts accurately in the designated systems
• Keep timely and comprehensive records to fulfill program requirements and compliance standards
• Comply with organizational policies, privacy regulations, and legal obligations
• Execute other responsibilities as assigned
• Understanding of pathophysiology and recognized treatment protocols for prevalent behavioral health disorders
• Capability to analyze patient records to pinpoint care gaps and report findings to the provider
• Ability to work autonomously in a remote setting without distractions
• Strong computer proficiency and adaptability to various technological platforms
• Exceptional written and verbal communication abilities
• Proven experience utilizing clinical data to inform decision-making
• Ability to operate independently as well as collaboratively within an interdisciplinary care team
• Must possess an active, unrestricted compact LPN or RN or MA license
• Capability to acquire additional state licenses as necessary
• Minimum of 2 years of experience in the healthcare field is required
• Experience in care settings may include inpatient, outpatient, or skilled nursing facilities
• Prior Case Management experience is preferred
• CCM certification is highly preferred
• Familiarity with Electronic Health Records is preferred
