This is a fully remote position, open to applicants in United States.
β’ Perform outreach to designated ECM members through phone calls, electronic messaging, or other approved methods.
β’ Support members in engaging with the ECM program and ensure consistent communication.
β’ Organize medical, behavioral health, and community-based appointments.
β’ Assist members in identifying and accessing healthcare providers and community resources.
β’ Connect members with resources related to behavioral health, housing, food, transportation, social services, advocacy, and more.
β’ Follow up on appointments, referrals, services, and care requirements.
β’ Relay member needs, obstacles, progress, and changes to Care Managers.
β’ Collaborate with healthcare providers, behavioral health specialists, community organizations, and social service agencies.
β’ Aid members in navigating healthcare and social service systems.
β’ Identify challenges in accessing or maintaining connections to services.
β’ Promote continuity of care and communication with providers and the care team.
β’ Complete necessary documentation for member interactions, outreach, referrals, coordination, and follow-ups.
β’ Maintain well-organized records of members, outstanding tasks, referrals, and follow-up requirements.
β’ Safeguard member confidentiality and adhere to privacy, organizational, and program protocols.
β’ Participate in team meetings, case coordination, training sessions, and program-related gatherings.
β’ Report safety, care, or service barrier concerns to the appropriate Care Manager or supervisor.
β’ Undertake additional ECM-related responsibilities as dictated by program and member needs.
β’ Preferred experience in care coordination, case management, behavioral health, healthcare, social services, community outreach, patient navigation, or a related field.
β’ Preferred experience working with individuals who have complex medical, behavioral health, or social service needs.
β’ Preferred experience in communication with healthcare providers, community organizations, or government agencies.
β’ Preferred knowledge of community resources and social service programs.
β’ Preferred experience in documenting client or member interactions and service coordination activities.
β’ Ability to manage multiple assignments and follow up on responsibilities.
β’ Ability to uphold confidentiality and maintain professional boundaries.
β’ Ability to work independently while keeping open lines of communication with the care team.
β’ Strong verbal and written communication skills.
β’ Ability to engage with individuals from diverse cultural, socioeconomic, and personal backgrounds.
β’ Organizational skills, attention to deadlines, and capacity to manage multiple members, appointments, referrals, and follow-up tasks.
β’ Comfort in communicating with healthcare providers, behavioral health professionals, community organizations, and social service agencies.
β’ Preferred bilingual or multilingual communication skills based on the needs of the populations served.
