This is a fully remote position, open to applicants in United States.
• Engage with assigned ECM members through telephone, electronic communication, or other authorized methods.
• Support members in their participation in the ECM program and ensure continuous communication throughout their involvement.
• Arrange medical, behavioral health, and community-based appointments.
• Assist members in identifying and accessing healthcare providers and community resources.
• Connect members with resources related to behavioral health, housing, food, transportation, social services, advocacy, and more.
• Follow up on appointments, referrals, services, and identified care needs.
• Relay member needs, obstacles, progress, and changes to Care Managers.
• Collaborate with healthcare providers, behavioral health professionals, community organizations, and social service agencies.
• Aid members in navigating healthcare and social service systems.
• Identify barriers impacting access to or continuity of services.
• Support the continuity of care and communication with providers and care teams.
• Complete necessary documentation for contacts, outreach, referrals, coordination, and follow-ups.
• Maintain organized records of assigned members, pending tasks, referrals, and follow-up needs.
• Safeguard member confidentiality and adhere to privacy, organizational, and program protocols.
• Participate in team meetings, case coordination activities, training sessions, and program-related gatherings.
• Report safety, care, or service-barrier concerns to the designated Care Manager or supervisor.
• Fulfill additional ECM-related responsibilities as dictated by program and member needs.
• Proficient verbal and written communication skills.
• Ability to interact effectively with individuals from various cultural, socioeconomic, and personal backgrounds.
• Strong organizational skills and attention to deadlines.
• Capability to manage multiple members, appointments, referrals, and follow-up tasks.
• Comfort in communicating with healthcare providers, behavioral health professionals, community organizations, and social service agencies.
• Preferred experience in care coordination, case management, behavioral health, healthcare, social services, community outreach, patient navigation, or a related field.
• Preferred experience working with individuals who have complex medical, behavioral health, or social service needs.
• Preferred experience in communicating with healthcare providers, community organizations, or government agencies.
• Preferred knowledge of community resources and social service programs.
• Preferred experience documenting client or member contacts and service coordination activities.
• Preferred ability to manage multiple assignments and follow-up responsibilities.
• Ability to maintain confidentiality and professional boundaries.
• Ability to work independently while keeping in contact with the care team.
• Preferred bilingual or multilingual communication skills based on population needs.
