This is a fully remote position, open to applicants in Pennsylvania.
• Acts as the primary point of contact between the patient, their family, and the clinical team for patients and their loved ones.
• Offers comprehensive support to collaborate with patients and families to manage, navigate, and arrange all facets of their healthcare.
• Responsible for fostering advanced-level partnerships with patients, including the scheduling of their appointments.
• Organizes and coordinates intricate appointments for patients throughout their care continuum.
• Evaluates the needs of patients and families, ensuring adherence to care plans.
• Engages in patient outreach to follow up on their care and appointment requirements.
• Communicates with patients through traditional and innovative channels, such as phone calls, patient portals, and other virtual platforms.
• Promotes effective communication among the physician/provider, clinical care team, as well as financial, outpatient, ancillary, and supportive services.
• High School Diploma/GED with a commitment to pursuing further education.
• A minimum of 2 years of experience engaging with patients as part of this role.
• At least 3 years of experience in a complex healthcare system environment or a related field.
• Exceptional accuracy and attention to detail.
• Ability to manage multiple tasks simultaneously.
• Outstanding customer service abilities.
• Basic computer functionality knowledge.
• Proficient and professional written and verbal communication skills.
• Capability to support patients and families during high-stress situations.
• Commitment to maintaining strict confidentiality.
