Care Coach

This is a fully remote position, open to applicants in United States.

📋 Description

• Assess member needs by addressing caregiver inquiries through portal outreach.

• Offer personalized coaching, emotional support, and guidance to caregivers during their caregiving journey.

• Inform members about available resources, services, and support networks in their local communities.

• Assist members with issues related to care recipients' health and wellbeing, including insurance, financial resources, Medicare, Medicaid, VA benefits and services, private insurance, employer-sponsored benefits, and public assistance programs.

• Respond to questions, provide information, and clarify resources involved in navigating healthcare, education, insurance claims, or legal systems pertinent to caregiving.

• Oversee caregiving logistics, such as identifying qualified caregivers, scheduling appointments, educating members on caregiving tasks, and supporting IEP/504 meetings.

• Address member inquiries or communications via phone or Cariloop's member portal within 2 hours of outreach.

• Collaborate with internal team members and external stakeholders, including healthcare providers, social workers, and caregiving professionals.

• Keep accurate and compliant records of coaching sessions, care support plans, communications, and resources in accordance with HIPAA and privacy regulations.

⛳️ Requirements

• An active license and/or certification in your field of expertise within your state of residency, such as LSW, LMSW, LCSW, LPN, RN, LNHA, CCM, COTA, OT, RT, or PT.

• A Bachelor's degree in nursing, psychology, social work, or a related field is preferred.

• Over 5 years of experience in providing caregiver support, managing care coordination, or working within a clinical, social work, or healthcare administration environment.

• Ability to engage with a diverse population, including individuals of varying age groups and those with disabilities or special needs.

• Proven capability to work independently, exercise sound judgment, and analyze complex or ambiguous situations with minimal guidance.

• Knowledge and understanding of the physical, financial, and mental health implications of caregiving, including stress, grief, and burnout.

• Proficiency in multitasking to efficiently manage workload.

• Capability to work from home with a designated space for phone communication.

• Bilingual proficiency in French or Spanish is an advantage.

• Must possess authorization to work in the United States.
