Care Advocate, LPN / Community Health Worker – Patient Navigation

This is a fully remote position, open to applicants in United States.

📋 Description

• Act as the main point of contact for patients following their intake appointment.

• Assist patients in comprehending and acting upon their care plans — including scheduling appointments, locating in-network providers, and utilizing their Medicare benefits.

• Address navigation challenges: portals, referrals, appointment logistics, forms, and communications with providers.

• Identify and connect patients with practical and social support resources (such as food, housing, transportation, etc.).

• Collaborate with clinical team members involved in a patient's care while offering non-clinical support exclusively.

• Accurately document every patient interaction and support activity in our platform in a timely manner.

• Ensure confidentiality and adherence to HIPAA, privacy regulations, and all relevant platform standards.

⛳️ Requirements

• One of the following: an active, unrestricted LPN/LVN license in good standing; a Community Health Worker (CHW) certification; or equivalent experience in community health or patient navigation.

• Over 2 years of experience in patient-facing or community-oriented support roles (such as care coordination, case management, member advocacy, health coaching, benefits navigation, community health work, or similar).

• Familiarity with the U.S. healthcare system and Medicare.

• Ability to work independently, manage a caseload, and maintain comprehensive documentation.

• Access to a reliable computer, internet connection, and a quiet environment for patient calls.

• Preferred: Experience working with older adults or patients dealing with complex or serious illnesses.

• Preferred: Knowledge of Medicaid and other public assistance programs.

• Preferred: Bilingual abilities in Spanish, Mandarin, or another language commonly spoken by Medicare patients.
