Cardiology Nurse Navigator

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Proactively engage with members, primarily through virtual platforms, to address clinical concerns and effectively manage any escalations that occur.

• Build trusting relationships with members and their care networks, emphasizing empathy and active listening in all interactions.

• Quickly identify and respond to member needs by providing support with care coordination, symptom management, nutritional guidance, discharge planning, and provider referrals.

• Under the supervision of our Nurse Team Lead or Nurse Practitioner, perform thorough clinical assessments, monitor member health, and facilitate care discussions aimed at enhancing quality, outcomes, and cost-effectiveness of care.

• Work in close collaboration with the Nurse Team Lead, Nurse Practitioner, and Medical Directors to ensure adherence to clinical protocols and best practices.

⛳️ Requirements

• Possession of an active Michigan RN license; licenses from surrounding states are an advantage.

• Bachelor of Science in Nursing and a compact unrestricted Michigan Registered Nurse license.

• A minimum of five years of nursing experience, including at least one year specializing in cardiology nursing.

• Experience in care management.

• Preferred experience in virtual or telehealth settings.
