This is a fully remote position, open to applicants in California.
• The Patient Record Abstractor IV plays a crucial role in accurately abstracting and coding clinical data from cancer patients' medical records according to the guidelines set by the California Cancer Registry (CCR), SEER Program, California Cancer Surveillance Program (CSP), UCLA Cancer Registry, and the Commission on Cancer.
• This role is vital in supporting UCLA Health’s dedication to data integrity, regulatory compliance, and cancer surveillance, ultimately enhancing patient outcomes, research efforts, and public health reporting.
• Abstract and code clinical and pathological information from medical records adhering to CCR, SEER, CSP, and Commission on Cancer protocols.
• Assign ICD-O codes, accession/sequence numbers, and guarantee precise data entry into the cancer registry database.
• Identify and assign the correct SEER Summary Stage.
• Conduct TNM staging for analytic cases.
• Assess Extent of Disease (EOD).
• Review and evaluate patient data in the electronic medical record (CareConnect) and paper charts as necessary.
• Validate clinical documentation to ensure the completeness and accuracy of the abstracted data.
• Abstract all necessary treatment details to guarantee prompt and comprehensive case reporting.
• Ensure that all reportable cases are finalized in accordance with registry standards.
• Maintain a high level of accuracy (≤3% error rate) following CCR standards.
• Achieve or surpass the established productivity benchmarks.
• ODS certification is mandatory.
• 5+ years of experience in abstracting.
• Proficient in using CNEXT, EPIC, and MS Office software (Constant).
• Experienced in operating computer equipment (Constant).
• Familiar with using the internet to locate sites and information essential for cancer reporting (Constant).
• Must fulfill productivity standards and State quality benchmarks.
• Comprehensive knowledge of the standards set by the American College of Surgeons, California Cancer Registry, and SEER.
• In-depth understanding of medical terminology, anatomy, pathology, and disease processes.
• Thorough knowledge of the ICD-0 coding classification system.
• Ability to accurately abstract clinical information from patient records and code data into the Cancer Registry database, following up for additional information when necessary.
• Exceptional organizational skills for maintaining Cancer Data files.
• Capable of prioritizing and managing a variety of tasks within a dynamic environment with changing priorities, assignments, and deadlines.
• Extensive knowledge of laws, rules, and regulations concerning the confidentiality of medical information.
• Proficient in working independently and completing assignments in an environment with frequent interruptions.
• Skillful in writing legibly and employing basic English for concise documentation.
• Understanding of the functions and activities of various hospital departments for efficient source information coordination.
• Ability to work autonomously and make appropriate decisions regarding follow-up actions, ensuring timely completion of assignments.
• Capability to assess system/procedure malfunctions or discrepancies and determine when to refer issues to a supervisor for immediate action.
• Strong oral and written communication skills, essential for conveying information to individuals at various organizational levels, the general public, and for completing concise documentation.
