Bilingual Spanish Telephonic RN Nurse Case Manager

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver telephonic case management for members with complex medical needs and chronic illnesses.

• Perform thorough health evaluations and develop personalized care plans.

• Collaborate with both internal teams and external partners, including physicians and specialists, to coordinate care.

• Provide education to members and their caregivers regarding disease management and preventive health measures.

• Track the progress of members and advocate for timely and appropriate care interventions.

• Identify and assist in resolving any service or access challenges that may affect the quality of care.

⛳️ Requirements

• Possession of an active, unrestricted RN license in California (Non-Compact).

• A minimum of 2 years of clinical nursing experience is required.

• At least 1 year of experience in case management is necessary.

• Proficiency in Microsoft Office applications (Word, Excel, Outlook) is essential.

• Bilingual abilities (Spanish, Korean, Mandarin, etc.) are considered a plus.

• Previous experience in a health plan or IPA is advantageous.

• A Bachelor's Degree in Nursing (BSN) is preferred.
