Bilingual – Spanish Speaking Licensed Practical Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform outbound telephonic outreach to patients and offer individualized coaching and support for those managing chronic illnesses, including but not limited to type 2 diabetes, hypertension, and cardiovascular disease.

• Assist patients in implementing tailored care plans and achieving their objectives by delivering education and coaching aimed at behavior modification, nutrition, physical activity, and self-management strategies, utilizing evidence-based methods such as motivational interviewing and SMART goals.

• Carry out thorough assessments of patients' health conditions, lifestyle choices, dietary habits, and willingness to make changes.

• Instruct patients on disease management, medication adherence, symptom identification, and prevention strategies, executing care delivery tasks in line with Cadence clinical policies and established protocols, all under suitable clinical supervision.

• Track patients' progress, adherence to their treatment plans, and health outcomes through regular follow-ups and remote monitoring tools.

• Record patient interactions, outcomes, and follow-up plans in the Cadence platform; identify barriers to care (including social determinants) and connect patients with resources or internal support channels.

• Collaborate in real time with clinicians and patient support teams using tools like our Cadence Platform, Google Workspace, and Slack, while providing feedback to enhance workflows in a dynamic, change-oriented setting.

⛳️ Requirements

• An active compact multi-state LPN/LVN license in the state of residence is mandatory.

• A minimum of 5 years of clinical experience as a Licensed Practical Nurse is required.

• Preparedness for remote work (including stable high-speed WiFi) is essential.

• An active CA state LVN license is strongly preferred.

• Proficiency in Spanish is a requirement.

• Experience in providing patient education and support for individuals managing chronic conditions is preferred.

• Familiarity with AI-assisted clinical tools (such as Gemini, Open Evidence, or similar platforms) and the capability to engage credibly with both providers and patients regarding the role of AI in care delivery and the indispensable nature of human judgment.
