Bilingual, Spanish Speaking Health Coach

This is a fully remote position, open to applicants in United States.

📋 Description

• Engage in proactive telephonic outreach to patients, offering individualized coaching and support for those managing chronic illnesses, including but not limited to type 2 diabetes, hypertension, and cardiovascular disease.

• Assist patients in implementing customized care plans and reaching their objectives through education and coaching that emphasizes behavior change, nutrition, physical activity, and self-management techniques grounded in evidence-based practices like motivational interviewing and SMART goals.

• Perform thorough evaluations of patients' health conditions, lifestyle choices, dietary habits, and their readiness to make changes.

• Instruct patients on effective disease management, medication adherence, symptom identification, and preventive strategies, while carrying out care delivery tasks in compliance with Cadence clinical policies and established guidelines, under suitable clinical supervision.

• Track patients' progress, adherence to treatment plans, and health outcomes through consistent check-ins and remote monitoring tools.

• Record patient interactions, outcomes, and follow-up strategies in the Cadence platform; recognize barriers to care (including social determinants) and link patients to resources or internal support systems.

• Collaborate in real-time with clinicians and patient support teams using tools such as the Cadence Platform, Google Workspace, and Slack, while providing feedback to enhance workflows in a dynamic, change-driven environment.

⛳️ Requirements

• An active NBHWC health coaching certification is mandatory.

• A minimum of 1 year of experience as a health coach is required.

• Proven experience in providing patient education and support to individuals with chronic conditions is essential.

• Remote work capability (including stable high-speed WiFi) is a necessity.

• Fluency in Spanish is required.

• Proficiency with AI-assisted clinical tools (such as Gemini, Open Evidence, or similar platforms) and the capacity to discuss the integration of AI in care delivery while understanding areas where human judgment is crucial.
