Bilingual Spanish Nurse Case Manager – Care Transitions, RN, CA License

This is a fully remote position, open to applicants in California.

📋 Description

• Develop tailored discharge plans in collaboration with patients, families, and the healthcare team.

• Facilitate care coordination among providers, rehabilitation facilities, and home health organizations.

• Track patient progress and modify plans as necessary.

• Instruct and empower patients and their families on self-care and follow-up measures.

• Advocate for patients' needs and link them to community support services.

• Engage in care conferences, quality improvement projects, and interdisciplinary teamwork.

⛳️ Requirements

• 2–3 years of experience in clinical care management (3–5 years preferred).

• Current, unrestricted RN license in California (willingness to obtain licenses for other states is required).

• Familiarity with Medicare Managed Care Plans, insurance regulations, and community resources.

• Excellent communication, problem-solving, and organizational abilities.
