This is a fully remote position, open to applicants in California.
β’ Develop tailored discharge plans in collaboration with patients, families, and the healthcare team.
β’ Facilitate care coordination among providers, rehabilitation facilities, and home health organizations.
β’ Track patient progress and modify plans as necessary.
β’ Instruct and empower patients and their families on self-care and follow-up measures.
β’ Advocate for patients' needs and link them to community support services.
β’ Engage in care conferences, quality improvement projects, and interdisciplinary teamwork.
β’ 2β3 years of experience in clinical care management (3β5 years preferred).
β’ Current, unrestricted RN license in California (willingness to obtain licenses for other states is required).
β’ Familiarity with Medicare Managed Care Plans, insurance regulations, and community resources.
β’ Excellent communication, problem-solving, and organizational abilities.
