Bilingual Spanish Health Coach, Educator

This is a fully remote position, open to applicants in California.

📋 Description

• Reach out to patients who qualify for Alignment’s Care at Home program and offer essential wellness coaching aimed at reducing or eliminating high-risk behaviors.

• Work in collaboration with advanced practice clinicians and/or social workers to create a comprehensive action plan.

• Promote the adoption of habits that lead to an improved quality of life.

• Ensure a high standard of patient service while enhancing member retention and satisfaction.

• Support patients in accessing community resources and social services to tackle social determinants that affect their health and well-being.

• Create and distribute health education materials to targeted patients, caregivers, and/or family members.

• Coordinate care with other Alignment team members, including health-related vendors, to ensure optimal patient care.

⛳️ Requirements

• Required: At least 1+ year of experience as a wellness or health coach or educator.

• Preferred: Experience in senior care.

• Required: High School Diploma or GED. An Associate’s degree or two years of additional experience may substitute for education.

• Preferred: Bachelor’s Degree.

• Required: Strong ability to communicate positively, professionally, and effectively with others; demonstrate leadership, and collaborate with team members.

• Comprehensive knowledge of specific diseases, lifestyle-related topics, nutrition, and exercise.

• Familiarity with the stages of change or a similar behavior change model.

• Exceptional interpersonal skills with the ability to build trust and respect with customers and colleagues.

• Capable of working independently and remotely.

• Proficient in handling protected health information while maintaining confidentiality and privacy in accordance with HIPAA regulations and AHP policies.

• Strong computer skills, including proficiency in Microsoft Office Excel, Word, and PowerPoint.

• Flexibility to work a varied schedule.

• Effective written and verbal communication skills; ability to cultivate and maintain constructive relationships with diverse members, management, employees, and vendors.

• Mathematical Skills: Competence in performing mathematical calculations and accurately calculating simple statistics.

• Reasoning Skills: Ability to prioritize multiple tasks, advanced problem-solving capabilities, and adeptness at using advanced reasoning to define problems, gather data, establish facts, draw valid conclusions, and develop, implement, and manage suitable resolutions.

• Problem-Solving Skills: Strong organizational and time management skills, with the ability to thrive in a fast-paced environment.

• Preferred: Bilingual in English and Spanish.
