Bilingual Care Manager – Telephonic Nurse

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Evaluates and assesses the needs and requirements of members to achieve and/or maintain an optimal state of wellness.

• Directs members and their families towards appropriate resources that support their care and wellbeing.

• Conducts health assessments, develops care plans, and provides education on chronic conditions and any gaps in their healthcare plans.

• Utilizes a range of strategies, approaches, and techniques to address a member's physical, environmental, and psycho-social health challenges.

• Identifies and addresses obstacles that impede effective care.

⛳️ Requirements

• Must be bilingual in English and Spanish.

• Bachelor's degree in nursing or a related health field.

• Must be a licensed Registered Nurse (RN) in the appropriate state with no disciplinary actions.

• At least 3 years of clinical acute care experience as a licensed RN.
